Could be lots of things, or more likely multiple things.  I'd suggest that 
you rig up a test headset.   Use a spare 1/4 inch jack and a pair of wires 
so that you can connect the tip of a headphone to one of the "G" terminals 
on the pre-amp and the ring of the headphone to the pre-amp chassis ground, 
and see whether you can hear anything in one of the earpieces directly out 
of the pre-amp, with the organ set for highest volume.  If you can, post 
back and we can trouble shoot the power amp.  If you can't hear anything 
(even a strong signal will be very low volume) then we can troubleshoot the 
organ, particularly the vibrato circuit.

Also, if you have access to a 12BH7 tube, try switching it into the pre-amp.
