So I found my glasses, took a look at the relay, and
it said 12VDC on the side. That's kind of an odd model number.

I'd been doing the rest of my testing with the amp in place.
There's an AC jumper in the speaker plug to keep you
from running the amp unloaded. I pulled the power tubes,
bridged the jumper, and fired it up on the bench.
I grounded pin 5 and I could hear the relay humming,
but it didn't trip. I poked it and away it went.
The contacts were just stuck.
Shot it with Cramolin,
burnished the contacts with paper,
put it all back together and away we go.
This little guy screams when you push him hard.

So, for the next guy looking for this information,
the Balwin Leslie BL-1 seems to use the following pinout:

1 AC hot
2 not used
3 unbalanced audio input
4 chassis ground
5 switch to ground to stop motor
6 AC neutral

I reversed the relay contacts,
so the relay's resting state is motor off.
I'm a momentary switch goose the Leslie once in a while kind guy.

There's room on the end face of the amp for a couple 1/4" jacks
for input and footswitch, so you just need to wire an AC cord
into the original plug. Very tidy.
