[HAM] A-100 Pedal Hardware Order?

I've got an annoying rattle at the joint between the metal extension  
arm and the phenolic arm to the doghouse, and I wanted to make sure  
I've got all the parts installed in the correct order. Could someone  
take a look for me :)

I've got: a screw, two metal washers, a metal spacer, a phenolic  
washer, and a nut.

Note to self: write this stuff down when you take something apart....
