[HAM] Arcing across pin 3 and 4 at leslie end, how common is this?

How common is it for pins 3 and 4 to arc and blow a fuse AND make a shit load of mess AND 
burn through the nearest faces of the bloody pins?
Twice this has happened to me now on separate Leslies.
I hate it when this stuff happens,
 it always makes me think I've wired something up wrong only to find(later) that I hadn't.
